NSW School Vaccination Program

Catch-up for all Year 8 students in 2022

All Year 8 students will be provided their 2nd dose of Human Papillomavirus (HPV) vaccine on Friday 25th February 2022, postponed during 2021 due to disruptions in the School Vaccination Program.

Please note to improve vaccination completion, students will be opportunistically offered catch-up vaccination for any missed doses, including HPV and dTpa vaccination throughout Year 8.

NSW Health will be using consent forms signed by parents/guardians during 2021. There is no need to complete a further consent form.

However, any parents/guardians who wish to withdraw their consent for any reason may do so by writing to the school Principal or phoning the school. The Procedure for Withdrawal of Consent is available on the NSW Health website at

www.health.nsw.gov.au/immunisation/Pages/withdraw_consent.aspx

A Record of Vaccination will be provided to each student vaccinated at each clinic. You should ensure that this record is kept for future reference. Please do not assume that your child has been vaccinated if you do not receive this Record of Vaccination. Vaccination records will be uploaded to the Australian Immunisation Register (AIR) to provide a complete immunisation history for your child.

Please ensure that your child eats breakfast on the day of the school vaccination clinic and brings plenty of food and water for throughout the day.

Should you have any further questions or comments regarding this program please call the Immunisation Team on 9840 3603 or call Ms Jones at Glenwood High School on 9629 9577.

Kind regards,

Ms Corrine Jones
